Self-Service Registration

Please fill out the form below to pre-register for access to the eligibility database.

This service is for providers needing to verify eligibility through Central Health’s Medical Access Program.  

Before your registration can be processed you will need read the Online Access Agreement for Provider Self Service and agree to the terms of the document by checking the “I agree to the conditions of the Online Access Agreement for Provider Self Service” box below. Failure to do so will disqualify your registration.
