Hornsby Bend Clinic
CommUnityCare offers primary care, preventive care, vaccines, lab services, chronic disease management, and limited pharmacy services at the new Hornsby Bend health center. The center also has a flexible space for other health services like behavioral health and/or health coverage enrollment.
Residents can schedule appointments on Mondays and Tuesdays from 10 a.m-7 p.m. and Fridays from 7 a.m-4 p.m.

The temporary house-calls pilot in Hornsby Bend will be ended with the opening of the new clinic. While current patients have been notified of the transition, some home-bound residents requiring home visits will continue to receive this service.
Colony Park & Creedmoor Mobile Clinic
Central Health’s new 40-foot mobile health clinic operates Monday through Friday, 8:30 a.m. – 4 p.m. at three locations.

– Tuesdays and Thursdays: Creedmoor Community Center, 12511 FM 1625 78610
– Wednesdays: Turner-Roberts Recreation Center, 7201 Colony Loop Dr. 78724
