HEALTH CHAMPION LOG IN

El Programa de Campeones de la Salud Comunitaria es un programa anual de Central Health que reúne a un diverso grupo de miembros de la comunidad de todo el Condado de Travis para aprender, discutir, y colaborar con nuestro trabajo para mejorar la salud del Centro de Texas.

A través de una serie de talleres de cinco meses de duración, los participantes aprenden acerca del sistema de cuidados médicos para los habitantes de escasos recursos económicos del Condado de Travis, y cómo Central Health y sus socios se unen para abordar las disparidades en salud.

Los Campeones de la Salud son desafiados a pensar fuera de sus realidades individuales para convertirse en defensores del acceso y la equidad en los cuidados médicos en sus propias comunidades.

QUIÉN DEBERÍA APLICAR

Si usted está comprometido con mejorar la salud de su comunidad, es un residente del Condado de Travis, y está interesado en aprender y aumentar sus conocimientos y su red personal en el campo de los cuidados médicos, lo invitamos a aplicar.

BENEFICIOS DE PARTICIPAR

  • Aprender de y formar redes con los líderes locales de los cuidados médicos
  • Convertirse en embajador de Central Health para mejorar la salud de la comunidad
  • Conocer de cerca la transformación de nuestro sistema de prestación de cuidados médicos
  • Fomentar relaciones para tener oportunidades futuras de colaborar en las principales iniciativas de la salud

COMPROMISO DE TIEMPO

Reunirse una vez al mes, de agosto a diciembre, para discutir una variedad de temas relacionados con las disparidades en salud y el sistema de cuidados médicos en el Condado de Travis. Los miembros deben comprometerse a participar en una capacitación en equidad de dos días de duración y en por lo menos tres de los cinco talleres anuales para recibir un certificado de participación.

Ricardo A. Garay

Ricardo A. Garay serves as a program manager for the community engagement and health equity team under the Department of Population Health. He also serves as a facilitator for Dell Medical School’s Community Strategy Team and serves as an advocate for community health workers in Central Texas.

Nora de Hoyos Comstock

Nora de Hoyos Comstock, Ph.D., is an entrepreneur, business leader, and pioneer in utilizing social media to build a community for Latinas. Nora believes that education, which translates into hope and opportunity, is the single most important factor that can transform lives.

Maram Museitif

Maram Museitif is a public health and health care professional with more than 10 years of experience working in large research, academic, and government health settings, and has a solid understanding of the challenges facing health care delivery in the United States and globally.

Amit Motwani

Amit Motwani was appointed to the Central Health Board of Managers by Travis County and the City of Austin in November of 2020.  His participation in Central Health's Community Health Champions program in 2017 provided him with a thorough understanding of the complexities around providing healthcare for Travis County residents with low income.

PROGRAM CURRICULUM

HEALTH EQUITY TRAINING

This training helps participants understand how historical and current cultural conditions, policies, and practices link to social inequities which link to health disparities. Through the training, Health Champions create an open environment where they can discuss concerns about and ideas for addressing systemic inequities and the impact on the local community.

WORKSHOP I

Introduction to the Central Health Enterprise

Health Champions will become familiar with the Central Health Enterprise, including the process of establishing coverage through Sendero Health Plans and Medical Access Programs (MAP and MAP Basic) as well as the eligibility criteria and benefits for these programs.

WORKSHOP II

Health Equity Plan & Budget

Health Champions will learn how Central Health partners with health care providers to ensure a robust network for the Medical Access Program (MAP and MAP Basic) members. This includes learning how Central Health identifies gaps and expands services for Travis County residents with low income. Additionally, this workshop provides insight on the upcoming fiscal year budget and its development process.

WORKSHOP III

Social Conditions

Health Champions will learn how the Central Health Enterprise addresses social conditions for Travis County residents with low income, and the many ways they can influence organizations like Central Health to better address health inequities either through their personal or their professional lives

WORKSHOP IV

Federally Qualified Health Centers

Health Champions do a deep dive on federally qualified health centers with a particular emphasis on CommUnityCare Health Centers and its most recent health equity initiatives.

WORKSHOP V

Community Outreach & Engagement

Health Champions will learn how Central Health works to engage directly with community members and involve residents in the planning, implementation and evaluation of Central Health projects, as well as keeping the community informed on services and health & wellness resources throughout Travis county.

Notice: The Travis County Healthcare District d/b/a Central Health adopted a tax rate that will raise more taxes for maintenance and operations than last year’s tax rate. The tax rate will effectively be raised by 6.0 percent and will raise taxes for maintenance and operations on a $100,000 home by approximately $5.47 (five dollars and forty-seven cents).
