Central Health Medical Access Program (MAP) is now scheduling in-person appointments for  individuals applying for MAP.  

· Call 512-978-8130 to schedule an appointment at one of the following locations – Central Health Southeast Health & Wellness Center located at 2901 Montopolis Dr. Austin, Tx 78741 or the Central Health Northeast Health Resource Center located at 6711 Johnny Morris Rd. Austin, Tx78724

· Individuals experiencing homelessness may be seen on a walk-in basis at Southeast Health and Wellness, all other applicants must call512-978-8130 to schedule an appointment

· Applicants can continue to apply online at www.centralhealth.net/get-started or over the phone by calling 512-978-8130
