Pulmonology Referral Guidelines for MAP Handbook

Brief summary of appropriate URGENT referrals:

  • Asthma – severe
  • Lung mass
  • Pulmonary embolism
  • SOB – severe

Brief summary of appropriate ROUTINE referrals:

  • Asthma – moderate, persistent, uncontrolled on meds
  • COPD
  • Cough – chronic
  • Interstitial lung disease (Pulmonary Fibrosis, Sarcoidosis, etc.)
  • Lung nodule
  • Neuro-related disorders with pulmonary complaint (ALS, Myasthenia Gravis, etc.)
  • Obstructive sleep apnea (must have current sleep study within 3 months of referral)
  • Pneumonia (unresponsive to initial treatment or complex)
  • Pulmonary hypertension
  • SOB

Please do NOT refer the following patients:

  • Asthma – mild intermittent, controlled on meds
  • Pediatric patients
  • Tuberculosis patients (should be referred to TB clinic)

Documentation required for scheduling an appointment:

  • Past Medical History (PMH)
  • Current medication list
  • Most recent progress note describing condition for which patient is being referred
  • Recent pertinent labs
  • Recent pertinent scans or imaging reports
  • Pulmonary Function Tests if available
  • Results of any skin tests performed (with dates and techniques)
