Health Plans \u6295\u4fdd\u79c1\u4eba\u5065\u5eb7\u4fdd\u9669\u7684\u673a\u4f1a\u3002<\/p>\n<p>\u622a\u81f3 12 \u6708 15 \u65e5\uff0c\u5df2\u6709 228 \u540d MAP \u548c\u6309\u6bd4\u4f8b\u6536\u8d39\u4f1a\u5458\u6ce8\u518c\u4e86\u65b0\u7684 Sendero \u8ba1\u5212\uff0c\u8be5\u8ba1\u5212\u88ab\u5f52\u7c7b\u4e3a\u975e\u4ea4\u6362\u8ba1\u5212\uff0c\u4e0d\u901a\u8fc7\u5065\u5eb7\u4fdd\u9669\u5e02\u573a\u63d0\u4f9b\u3002\u4e2d\u592e\u533b\u7597\u96c6\u56e2\u5c06\u627f\u62c5\u4ed6\u4eec\u5728\u65b0\u8ba1\u5212\u4e2d\u7684\u62a4\u7406\u8d39\u7528\u548c\u4fdd\u9669\u8d39\u3002\u901a\u8fc7\u5c06\u60a3\u8005\u4ece MAP \u548c SFS \u8f6c\u81f3 Sendero\uff0c\u4e2d\u592e\u4fdd\u5065\u4e2d\u5fc3\u53ef\u4ee5\u5229\u7528\u5730\u65b9\u7a0e\u6b3e\uff0c\u901a\u8fc7\u300a\u53ef\u8d1f\u62c5\u533b\u7597\u6cd5\u6848\u300b\uff08ACA\uff09\u7684\u98ce\u9669\u8c03\u6574\u8ba1\u5212\u4e3a\u60a3\u8005\u63d0\u4f9b\u533b\u7597\u670d\u52a1\u3002\u8be5\u8ba1\u5212\u7684\u76ee\u7684\u662f\u5bf9\u90a3\u4e9b\u75c5\u60c5\u8f83\u91cd\u3001\u9700\u8981\u66f4\u591a\u670d\u52a1\u7684\u4f1a\u5458\u8fdb\u884c\u8865\u507f\uff0c\u56e0\u4e3a\u8fd9\u4e9b\u4f1a\u5458\u7684\u4fdd\u9669\u8d39\u7528\u8f83\u9ad8\u3002<\/p>\n<p><a \/><\/a><\/p>\n<p>\u4e2d\u592e\u533b\u7597\u4e2d\u5fc3\u4e0e Seton \u7684\u975e\u8425\u5229\u6027\u5408\u4f5c\u4f19\u4f34\u793e\u533a\u533b\u7597\u5408\u4f5c\u7ec4\u7ec7 (Community Care Collaborative, CCC) \u5408\u4f5c\uff0c\u786e\u5b9a\u4e86\u60a3\u8005\u7684\u8eab\u4efd\u5e76\u5206\u6790\u4e86\u4ed6\u4eec\u7684\u98ce\u9669\u8bc4\u5206\u3002\u98ce\u9669\u8bc4\u5206\u53cd\u6620\u4e86\u60a3\u8005\u56e0\u5065\u5eb7\u72b6\u51b5\u3001\u533b\u7597\u7cfb\u7edf\u4f7f\u7528\u60c5\u51b5\u548c\u5176\u4ed6\u8003\u8651\u56e0\u7d20\u800c\u9762\u4e34\u7684\u6b7b\u4ea1\u6216\u75be\u75c5\u98ce\u9669\u6c34\u5e73\u3002\u622a\u81f3 11 \u6708\uff0c\u4e2d\u592e\u4fdd\u5065\u4e2d\u5fc3\u5df2\u786e\u5b9a\u4e86 1085 \u540d\u7b26\u5408\u65b0\u8ba1\u5212\u8d44\u683c\u7684\u60a3\u8005\u3002\u4ed6\u4eec\u7684\u98ce\u9669\u5206\u6570\u5927\u5927\u9ad8\u4e8e\u5dde\u5e73\u5747\u6c34\u5e73\u3002<\/p>\n<p>\u6570\u5b57\uff1a\u68ee\u5fb7\u7f57\u4e2d\u592e\u533b\u7597\u4fdd\u9669\u8d39\u8865\u52a9\u6269\u5c55\u8ba1\u5212<br \/>\n\u5171\u6709 228 \u540d MAP \u6216 Community Care Collaborative (CCC) Sliding Fee Scale \u4f1a\u5458\u9009\u62e9\u901a\u8fc7 Sendero \u52a0\u5165\u4e2d\u592e\u4fdd\u5065\u4fdd\u8d39\u8865\u52a9\u6269\u5c55\u8ba1\u5212 (CHAP)\u3002\u4e2d\u592e\u4fdd\u5065\u5728 19 \u8d22\u653f\u5e74\u5ea6\u7684\u4fdd\u8d39\u6210\u672c\u7ea6\u4e3a $3 \u767e\u4e07\u7f8e\u5143\u3002<\/p>\n<p>\u5916\u8054\u5de5\u4f5c\u5305\u62ec<br \/>\n- 1 730 \u6b21\u7535\u8bdd\u547c\u53eb<br \/>\n- \u90ae\u5bc4\u4fe1\u4ef6 770 \u5c01<br \/>\n- 123 \u6b21\u5bb6\u8bbf\u5c1d\u8bd5<\/p>\n<p>\u80cc\u666f\u4ecb\u7ecd<br \/>\n\u5c06\u7b26\u5408\u6761\u4ef6\u7684 MAP \u548c SFS \u60a3\u8005\u8f6c\u81f3 Sendero \u662f\u4e2d\u592e\u533b\u7597\u96c6\u56e2\u975e\u8425\u5229\u533b\u7597\u4fdd\u9669\u516c\u53f8\u8fd1\u671f\u8d22\u52a1\u7a33\u5b9a\u6218\u7565\u7684\u4e00\u90e8\u5206\u3002\u4eca\u5e74 9 \u6708\uff0c\u4e2d\u592e\u5065\u5eb7\u7ba1\u7406\u59d4\u5458\u4f1a\u6295\u7968\u51b3\u5b9a\u5728 2019 \u8d22\u5e74\u4e3a Sendero \u63d0\u4f9b $26 \u767e\u4e07\u7f8e\u5143\u7684\u989d\u5916\u8d44\u91d1\u3002<\/p>\n<p>Sendero \u5fc5\u987b\u5b9a\u671f\u5411\u4e2d\u592e\u5065\u5eb7\u7ba1\u7406\u59d4\u5458\u4f1a\u63d0\u4f9b\u6210\u679c\u63aa\u65bd\uff0c\u4ee5\u8bc1\u660e\u8be5\u6218\u7565\u662f\u6709\u6548\u7684\uff0c\u5426\u5219\u59d4\u5458\u4f1a\u5c06\u5bf9\u672a\u6765\u7684\u8d44\u91d1\u6295\u5165\u8bbe\u7f6e\u4e0a\u9650\uff0c\u8fd9\u5f88\u53ef\u80fd\u8feb\u4f7f Sendero \u9000\u51fa\u4fdd\u9669\u5e02\u573a\u3002<\/p>\n<p>\u8be5\u8ba1\u5212\u7684\u8d44\u52a9\u671f\u9650\u4e3a\u4e00\u5e74\u3002\u5982\u679c\u4e00\u5e74\u540e\u8ba1\u5212\u8d44\u91d1\u4e0d\u518d\u5230\u4f4d\uff0c\u7b26\u5408\u6761\u4ef6\u7684\u53c2\u4e0e\u8005\u53ef\u9009\u62e9\u91cd\u8fd4
