How Connected Care is Rewiring Austin’s Affordability Crisis

Marlene Rosabel Conde understands this vulnerability all too well.

Not long ago, the Austin resident faced a sudden health scare that turned into an emergency hospital visit, forcing her to navigate an out-of-network system. The true gut-punch was the $1,700 bill that followed. Unsure of where to turn, she was unable to pay.

“I didn’t have anything to pay my rent,” Rosabel said through a translator. “We live in fear with the cost of medical bills.”

The Cost of Disconnected Care

Rosabel Conde’s experience is not an isolated incident. Her story represents a fundamental threat to the region’s economic stability. When people cannot access care early due to cost, confusing referrals, or long waits, manageable conditions can quickly become life-threatening emergencies.

This creates a devastating ripple effect. For vulnerable residents, lost wages from missed work can spiral into severe, untreated health crises, sometimes leading to tragedy. But the impact does not stop there. Unmanaged health disparities are estimated to cost the Travis County economy approximately $1.1 billion annually.

This $1.1 billion is a “hidden tax” that creates a trickle-down effect on the rest of the community. Preventable emergency room visits jam local hospitals, while sick employees missing work heavily impacts overall economic productivity.

Creating Seamless Care Journeys

To solve these systemic challenges, Central Health—Travis County’s public health care system—is working to address the root causes. While building and expanding a health care safety net requires public support and taxpayer investment, Central Health is approaching this work as essential economic infrastructure designed to act as a shock absorber for Austin’s affordability crisis.

The cornerstone of this shift is a massive administrative overhaul. To curb the waste often associated with fragmented public services, Central Health has consolidated its operations with CommUnityCare Health Centers and Sendero Health Plans into an integrated Central Health system to reduce duplication and waste. By consolidating vendors, streamlining contracts, and sharing administrative services, the Central Health system is demonstrating that public health can operate with the lean efficiency of a private enterprise, but remain dedicated to a mission of connecting people to care and ensuring public dollars go further by preventing avoidable crises.

This strategy is anchored by a primary objective of creating seamless care journeys for patients. At its core, this objective is about making it easier for patients to get the right care at the right time. The Central Health system is building a more connected public health care system so people can move from coverage to clinics, from primary care to specialty care, and from hospital care to recovery with fewer barriers.

Practically, creating a seamless journey means ensuring a referral is never a dead end. Central Health is aggressively expanding its direct clinical practice in high-demand specialties—such as cardiology, general surgery, and physical therapy—to drastically cut appointment wait times down to two weeks or less. It also means bringing services together. By fully operationalizing joint facilities like the flagship Hancock Center, patients of the Central Health system can soon access primary care, specialty services, adult dental, and a pharmacy all under one roof.

Tackling Affordability Through Coordination

This seamless approach directly tackles the region’s affordability crisis. By strengthening care coordination and patient navigation—including scheduling assistance, contracted transportation services, and housing placement support—Central Health is actively removing the logistical barriers that delay care. By embedding social workers into skilled nursing facilities and expanding targeted primary care teams for justice-involved and unhoused residents, the system provides a safety net for the community’s most vulnerable.

The Central Health system is demonstrating the best way to solve long-term affordability problems is to build a system that handles health issues before they become expensive emergencies. When care happens earlier and works better together, public dollars go further, and the cost of living is ultimately lowered for everyone in Travis County.

A Community-Shaped Future

Accessible health care functions as a regional utility, safeguarding both the local workforce and taxpayer investments. But achieving a truly connected system requires ongoing community input to identify what is still missing from health care in Travis County.

For Rosabel Conde, finding a connected support system made all the difference.

In the weeks following her health scare, her fears around cost soon began to fade as Central Health system outreach workers and case managers listened to her. Sendero Health Plans helped Rosabel navigate and resolve her medical bill and identified resources to address her broader needs.

“I found organizations and people that listen to me, that offered help,” she said. “More than anything, I’m very proud of this community.”
