We will also create a wound care plan customized only for you. This plan will deal with things like how well you can move, your cognitive ability, living situation, transportation needs, the foods you eat, and your support system. We will always plan around your goals for your own healthcare.

Central Health, Travis County’s Hospital District, provides these specialty care services to members of the Medical Access Program (MAP) and MAP Basic after referral from a primary care provider.
