MAP and MAP Basic connect members like you to local doctors, dentists, pharmacies, and specialists. After your first doctor’s appointment as a MAP member you can access your medical records, test results, and make future appointments by logging into your MyChart Account.

Group/Plan: Your MAP or MAP Basic plan you are enrolled in

EFF: The date your health care services start. Stands for “effective”

EXP: The date your membership expires.

Name: Your name

DOB: Your Date of Birth.

PCP: The name of your Primary Care Provider or Clinic and their phone number

PCP/SPC/UC $XX: Primary Care / Specialty Care / Urgent Care with copayment amounts

OP/ER/IP: Outpatient, Emergency Room, Inpatient with copayments amounts

RX: Pharmacy / prescription with copayment amoiunts

DEN/SPDEN: Dental, Specialty Dental with copayment amounts

RXBIN/PCN/GPN ID: For pharmacy use only

Helpful tips
  • Always show your membership card and photo identification when you check in for an appointment or pick up a prescription. Your clinic and pharmacy also need to know when you have a new membership card.
  • You are the only one who can use your membership card.
  • You could lose your services from Central Health if you let someone else use your card.
  • If you have a friend or family member who needs health care, they can apply for MAP and MAP Basic online at centralhealth.net/map or call 512-978-8130 for help.

Schedule your first appointment

The first step as a new member is to schedule an appointment with a primary care provider (PCP).

Your primary care provider (PCP) is the doctor, nurse practitioner, or physician assistant you will see first for your medical care. They will make sure all your health care providers, including specialists, work together.

Call the clinic’s phone number on your membership card to schedule a new patient visit as soon as possible.

Helpful tips
  • Are you sick before your first visit?
    You can visit an Urgent Care Clinic in our network.
  • You selected your clinic when you enrolled.
  • Don’t wait, it may take a couple of months to be seen as a new patient.
  • Get Regular Checkups also known as wellness visits.
  • It’s important to get a checkup every year, even if you’re not feeling sick.

Urgent care

An urgent care center is an option for minor illnesses and injuries such as:

  • colds, flu, and allergies
  • sore throats
  • fever
  • earaches
  • vomiting, nausea, or diarrhea
  • rashes and skin infections
  • sprains and strains
  • minor bone fractures
  • pink eye
  • small cuts requiring stitches

Urgent care centers often can also do lab tests and X-rays. You can walk in without an appointment. Many of these centers have evening and weekend hours.

Dental

Healthy teeth and gums are an important part of your health and wellbeing. MAP, MAP Basic, and MAP Basic Dental Only cover many important dental services, including:

  • Regular dental exams and cleanings
  • Periodontal services (gum care)
  • X-rays of your teeth
  • Fillings for cavities
  • Tooth extraction (removal)
  • Dentures (false teeth)
  • Oral surgery

Dental services are offered at CommUnityCare, Lone Star Circle of Care, People’s Community Clinic, and Manos de Cristo. Call the number on your membership card to make an appointment.

Pharmacy and medications

MAP and MAP Basic cover pharmacy services, including prescriptions and disposable medical supplies like insulin syringes.

Based on your membership plan your copay may be up to $30. Review your membership card to see the amount you will pay in the line called RX: Form/Non-Form $X/$X.

Always show your membership card when you get a prescription. Your doctors also need to know when you have a new card.

It is important to talk to your provider about where to pick up your prescription.

  • Cardiology: Care for people with heart problems.
  • Dermatology: Care for people with skin problems.
  • Dietitian services: Help from experts to improve your diet.
  • Ear, Nose, and Throat (ENT): Care for people with ear, nose, or throat problems.
  • Gastroenterology: Care for people with stomach and gut problems.
  • Hepatology: Care for people with liver problems.
  • Hospice and palliative care: Help to feel better when you have a serious illness.
  • Infectious disease management: Care for people with illnesses that can be spread to others.
  • Musculoskeletal Care (also known as MSK or Orthopedics): Care for people with problems related to their muscles and bones.
  • Nephrology: Care for people with kidney problems.
  • Oncology: Care for people with cancer.
  • Oral surgery: surgical services for people with dental problems.
  • Physical Medicine & Rehabilitation (PM&R): Care for people to restore functional abilities and improve quality of life.
  • Podiatry: Care for foot and ankle problems.
  • Psychiatry: Care for people who need help with mental health problems.
  • Pulmonology: Care for people with breathing problems.
  • Rheumatology: Care for people with joint problems.
  • Substance Use Disorder Treatment: Care for people with opioid addiction.
  • Urology: Care for people with urinary tract infections.
  • Wound Care: Care to help manage injuries and promote healing.

Emergency care

If you need medical emergency services, call 9-1-1 or go to the closest Emergency Room.

How do I renew?

Renew your membership card before it expires so you always have coverage. Keeping your coverage active is the best way to make sure you get the care you need when you need it.

Renewing your membership is easy.

  • GATHER the documents you need (listed below this section)
  • SUBMIT your application online or call 512-978-8130 to get started.
  • OR APPLY in person at several locations across Travis County. Call 512-978-8130 to schedule a MAP application appointment.
  • WATCH for important updates from Central Health. These can include mailed letters, phone calls, emails, or text messages.
Helpful tips
  • Start your renewal process at least 30 to 45 days before your card expires.
  • MAP card members renew every 6 months.
  • MAP Basic card members renew every 12 months.
  • Look next to EXP to find out when your MAP or MAP Basic card expires.
  • Opt-in for text messaging reminders

Our new look

At Central Health, we make sure you can get the health care you need, wherever you are.

We’re happy to announce we have a new look for the Central Health system that shows how we are all united in our promise to give you the best care possible.

How will this new logo affect you? It will make it easier for you to see Central Health, CommUnityCare Health Centers, and Sendero Health Plans are parts of the same health care system.

Even though we are all getting a makeover, the services we offer you won’t change. You will not need to do anything different. Just know the Central Health system is working together to make health care better for everyone in Travis County.

Changes you will see:

This fall, the logo and colors on the MAP and MAP Basic cards will change. Don’t worry, your current card is good until it expires. Your new card will have all the same info as your old one.

Over the next year, our new logo will begin to show up on emails, letters, websites, and social media.

Over the next two years, our new logo will be on our employees’ uniforms and on clinic signs.

Our new logo comes from talking to many MAP members like you. It stands for compassion, commitment, trust, and working together. It is bright and colorful to show we are modern and always ready to help.

When you see our new logo, it will be clear the Central Health system is working together to take care of you. If you are signing up for MAP, seeing your doctor, or getting your medications, know we are here for you.
